I've always used MS Paint and then a little MS Office add-on program for transparency, and maybe a greeting card editor program from a CD for fancy stuff (not often).

Most users use Paint.net though.

Don't forget Photoshop, although most of it's effects are voided due to the fact that the template is very small.

Paint.net because it can do transparent. But Simple MSPaint would do good.
